A scoping review on the mental health harms of LLM-based chatbots - npj Digital Medicine - Nature
IntroductionArtificial conversational agents (e.g., chatbots) share a long history with mental health practices. In the 1970s, Joseph Weizenbaum developed ELIZA, a rule-based chatbot that could detect key words in input text to phrase open-ended questions, which has found anecdotal use as a mental health support chatbot1. Although Weizenbaum advocated against the use of chatbots for the treatment of mental disorders2, mental health chatbots have evolved over time, with rule-based or hybrid chatbots increasingly used for mental health support3,4,5,6,7. Nevertheless, rule-based chatbots’ potential for personalized engagement styles remained limited by their lack of flexibility and adaptability. This paradigm shifted with the development of large language models (LLMs): language processing systems based on artificial intelligence (AI) capable of producing natural-sounding human language and conversations.
