Postpartum “Hypercoagulable Storm” With Extensive Iliocaval Thrombosis and Pulmonary Embolism in a Patient With Heterozygous Factor V Leiden - Cureus
A 26-year-old woman presented to the emergency department four weeks after a lower segment cesarean section with a four-day history of progressive dyspnea, bilateral lower limb swelling, and leg pain. At presentation to the emergency department, she was tachycardic (150 beats/min), hypotensive (88/60 mmHg), and tachypneic (38/min), with an oxygen saturation of 93% on room air. Venous Doppler ultrasonography demonstrated extensive bilateral deep vein thrombosis (DVT) involving the iliac veins, right external iliac vein, infrarenal inferior vena cava (IVC), and right popliteal vein, with left popliteal involvement. Computed tomography (CT) pulmonary angiography demonstrated acute pulmonary embolism (PE) involving the right pulmonary artery and multiple segmental branches, with right lower lobe pulmonary infarction. Thrombophilia evaluation identified heterozygous Factor V Leiden, with negative antiphospholipid antibodies and lupus anticoagulant and normal protein C, protein S, and antithrombin III levels. The initial presentation raised concern for high-risk acute pulmonary embolism physiology, with hypotension (88/60 mmHg), marked tachycardia, and respiratory compromise requiring hi

Pediatric Pituitary Neuroendocrine Tumor With Extensive Necrosis: A Case Report - Cureus
We describe an unusual presentation of a pediatric pituitary neuroendocrine tumor (PitNET). A 17-year-old previously healthy male presented to the emergency department with altered mental status, lethargy, nausea, vomiting, fever, and tachycardia. Head imaging incidentally demonstrated a large expansile sellar/suprasellar cystic and solid mass displacing the optic chiasm, invading bilaterally into the cavernous sinuses, and encasing the internal carotid arteries. He developed decreased vision of the left eye on hospital day two and underwent urgent endoscopic endonasal transsphenoidal resection of the mass. Pathology demonstrated a PitNET with extensive necrosis, lactotroph differentiation, and changes suggestive of superimposed tumoral infection. His postoperative course was complicated by panhypopituitarism, chronic diabetes insipidus, and a cerebrospinal fluid leak managed with lumbar drainage. His presenting symptoms resolved completely, with no tumor recurrence at two-month follow-up. The initial clinical manifestation in this case was suggestive of adrenal crisis secondary to pituitary hypofunction and secondary adrenal insufficiency, attributed to significant tumor compressi

Prehabilitation program reduces death risk for frail transplant candidates - News-Medical
When transplant candidates become frail, doctors often worry that they are too weak to survive transplantation. But according to a new study from Northwestern Medicine and the University of Pittsburgh, frailty can be significantly improved with the right personalized, physician-led fitness program. In the study, more than 350 liver transplant candidates participated in a highly individualized workout and nutrition program. Nearly two-thirds of patients who attended at least three program visits lowered their frailty score by an average of nearly 0.4 points - a number which turned out to be a game changer.
